Who We Are

The Peninsula Model for Primary Health Planning is a catchment-based partnership. Now coordinated under the leadership and direction of the Frankston Mornington Peninsula Primar Care Partnership (FMPPCP), it involves a range of health and community service organisations, key stakeholders, consumers, carers and communities.

The FMPPCP is a founding partner of The Peninsula Model. Whilst acting as the 'backbone organisation' for the Peninsula Model, health promotion, integrated chronic disease management, community engagement and service coordination remain core strategies and responsibilities of FMPPCP. These core functions are integrated and embedded with the work undertaken through the Peninsula Model.

Partnership and participation in activities of the Peninsula Model and membership of FMPPCP comprises health and community service organisations committed to strengthening the planning, coordination and delivery of services by using a range of innovative and collaborative strategies. These strategies are designed to improve the outcomes of people using health and community support services across the catchment


Further to the direct involvement of dozens of health and community support services and private providers of the Peninsula Model, the FMPPCP is made up of 38 member organisations.
